two year old bill still unresponsive to my request for an explanation of benifits as required by supplementary..i have concluded take over by lee memorial of gulf coast caused a loss of their records since they (united health),had paid and therefore have no reason to hold up the process. also since there is no other reason to deny this form i must conclude this a fraudlant process.
